In-Home Supports
At QHH, our aim is to assist participants to live in their own home and access their local community, as independently as possible. We offer in-home supports and services to suit young people and adults alike, delivered by a team of trained, vetted, and experienced professionals. Whether participants need assistance with domestic tasks like cooking, cleaning, or gardening; personal care tasks like showering, dressing, or moving around; or going out into the community for shopping, socialising, or appointments, we are here to lend a hand! We partner with participants and families to get to know them and understand their background, circumstances, and preferences, to deliver the support they want. Support Coordination & Connection
Our support coordinators can assist participants to understand and implement their NDIS plan. QHH support coordinators are highly trained professionals who specialise in assisting participants to connect with mainstream, NDIS and community supports that align with their plan goals. We pride ourselves on ensuring participants are empowered to exercise choice and control over the supports they access, and we assist them to learn skills to be more independent in coordinating their services. To access support coordination and connection services, participants need support coordination funding built into the capacity building section of their NDIS plan. Centre-Based Programs
Our Centre-Based Programs aim to provide opportunities for people with intellectual disabilities to learn practical and soft skills required for more independent living. Our programs cover different aspects of daily living, including employment, health and well-being, accessing the community, domestic skills, and literacy. Each program is run in small groups, and is tailored to participant goals. There is no set timeframe for attending our programs, as they will be paced to suit the needs of each participant.
